Nurse Practitioners are educated to degree level to achieve a high standard of patient care. They are qualified to assess patients' physical, psychological and social needs and to diagnose and implement agreed management plans with patients, including appropriate prescribing and referrals to specialists.
We have five practice nurses who all have additional specialist training in primary care. As well as performing the usual practice nurse procedures they have been trained to monitor specific diseases such as asthma, diabetes and coronary heart disease. Health promotion and health education forms a large part of the work of our nurses and can help patients who would like to improve their health, for example by weight reduction or help to stop smoking.
The Clinical Practitioner assists the GP's at the practice with home visits.
We have three healthcare assistants and one phlebotomist who can undertake a number of general nursing procedures including blood tests, blood pressure recording, wound care, general health checks, ECGs and smoking cessation advice.
IT/Admin Team Leader
Kirsty has responsibility for managing the administration department as well as all information technology services within the practice.
Assisting practice manager and IT lead.
We have a large team of Administrators including 3 Medical Secretaries, who deal with the ever-increasing paperwork associated with maintaining a high demand service. A key function of this department is electronic data management and maintenance of a large clinical computer system.
The Practice has a large reception team who together each week between the hours of 07:45 - 19:00, process more than 1400 repeat prescriptions and make appointments for and greet around 2500 patients per week as well as handling vast numbers of telephone enquiries.
The district nursing team are employed by The Ipswich Hospital and are based at Ravenswood Medical practice.
Our main aim is to provide skilled nursing care to patients who are truly housebound e.g. cannot go out shopping, go to the hairdresser or to bingo etc.
This includes managing chronic illness, such as diabetes, palliative/terminal care, leg ulcer management, venepuncture, continence care, pressure area care, support and advice and health promotion.
Our service provides evidence based care, promoting independence where possible. We work closely with the GPs and attached health professionals to provide a holistic service to our patients.
Referrals to the district nursing team service are made via the Care Coordination Centre. All referrals to the team can be made by GPs, hospitals, social services, work colleagues, patients, relatives, or carers.
